- Performs an assessment and develops a specific Plan of Care for each patient that focuses on achievable patient outcomes.
- Initiates music therapy programs and instructs other personnel and/or family/caregivers how they may work with the patient.
- Plans, organizes and implements Music Therapy for individuals and groups, for sensory-stimulation, pain diversion, life review Completes an evaluation to determine if the goals for patient have been met.
- Documents initial evaluations, progress notes, patient/family responses to music therapy and outcomes from music therapy.
- Collaborated with other team personnel to achieve well-rounded care.
- Degree in Music Therapy from an AMTA
- Board Certified Music Therapist (MT-BC) or certification within 90 days of employment.
- 6 month internship Music therapy Internship (from either national roster or approved university affiliation.
- Experience with Music Therapy in a hospice, geriatric, or hospital setting preferred
- 1-2 years' experience in related field. Previous experience in home health, pediatrics, and/or hospice preferred.

Tidewell Hospice, the sole provider of hospice services in four counties of sunny Southwest Florida, is seeking a Music Therapist.
The Music Therapist plans, organizes, and facilitates approved music therapy programs for hospice patients in homes, Long Term Care facilities (LTC), Assisted Living Facilities (ALF), and hospice houses in the Venice/Sarasota area.
