We're looking for a dynamic individual who can also wear the hat of a business developer - someone who can nurture and grow our partnerships with healthcare facilities through strategic relationship-building and effective communication.

You have a Valid LPN or RN license in New Jersey and 3+ years of experience in a nursing facility or similar healthcare environment.

Your background in care management shines, especially in coordinating patient care post-discharge.

Identify and coordinate the arrangement of patient support services, using your care management expertise to address post-discharge needs.
