- Provides non-clinical paraprofessional duties and is thoroughly familiar with the patients' community and the resources available.
- Co-facilitates group prenatal care (GPC) visits with a Women's Health provider to birthing people recruited to participate in this innovative prenatal care format.
- Conducts 1-on-1 visits with patients to address social determinants of health via the WIN Network curriculum model.
- Is responsible for effectively communicating outcomes and data with their fellow CHWs as well as the clinical care team.
- Follows up with patients via phone, home visits, and visits to other settings where patients can be found to assure comprehensive and coordinated care.
- Provides crisis intervention support to patients by identifying issues related to patient health, to assist patients in securing and following through on mental and physical health care and assess crisis situations and intervene as appropriate to ensure patient safety.
- Assists patients in navigating health care systems which may include transporting them to appointments.
- Utilization management and patient care coordination in conjunction with hospital emergency department, health plans, etc.
- Works cooperatively with other staff including providers in coordinating patient care and pursuing team-based approaches to service delivery.
- Engages patients/families in the development/maintenance and activation of a comprehensive treatment and self-management plan.
- Assists with building relationships in the community with outreach and other similar services.
- High School diploma or G.E.D equivalent
- Certification as a Community Health Worker or willingness to become certified.
- Previous experience conducting or facilitating groups.
- Previous experience conducting home and/or clinic visits with patients and their primary care providers preferred.
- Adequate knowledge of the various resources available in the community for CFH clients.
- Excellent communication skills both written and verbal.
- Ability to interact respectfully with diverse cultural and socio-economic populations.
- Ability to work effectively and independently with clinical and administrative staff as well as maintain external communications throughout the community.
- Ability to represent the agency effectively in a variety of settings and with diverse communities. Demonstrates understanding and appreciation for diverse cultures.
- Ability to work with MS Office Products software programs.
- Routine testing/training on various job-related skills and competencies.
- Must have a valid driver's license, automobile insurance, and a reliable vehicle.
- Chauffer's license or willingness to obtain one.
