    Description

    We are looking for a dedicated Caseworker to join our patient care team. As a Caseworker, you will play a vital role in providing direct assistance to patients and their families within the hospice program of care.

    • Minimum of two (2) years of experience in a health-care environment focusing on community resource utilization and patient interaction.
    • Must have reliable transportation with adequate insurance coverage or the ability to handle the emotional challenges of dealing with loss.
    • Thorough knowledge of community resources in the hospice's specific location.
    • Familiarity with local, county, state, and federal assistance programs.
    • Capacity to work independently with minimal supervision.
    • Willingness to learn and proficiently use company software programs.

    Qualifications:

    • Bachelor's Degree in Social Work or a related field from an accredited institution.
