Join Millennium Home Care, an affiliate of Millennium Healthcare, LLC, as a PRN Physical Therapist and make a meaningful impact on the lives of individuals in need of home care services. We are seeking a dynamic and compassionate professional to join our dedicated team.
Responsibilities:
Conduct comprehensive evaluations of patients' physical conditions, including assessment of mobility, balance, strength, and pain levels
Develop personalized treatment plans based on patients' specific needs and goals
Implement evidence-based therapeutic interventions to improve patients' physical function and reduce pain
Provide hands-on physical therapy to patients in their homes, ensuring their safety and comfort
Monitor patients' progress and adjust treatment plans as necessary
Collaborate with other healthcare professionals, such as physicians and nurses, to ensure coordinated and effective care
Educate patients and their families on proper techniques for exercises and activities to promote independence and recovery
Maintain accurate and up-to-date documentation of patient evaluations, progress, and treatment plans
Stay current with advancements in physical therapy through continuing education and professional development opportunities
Requirements:
Current state license as a Physical Therapist in Florida.
A Minimum of 1 year of experience in home care, or a related field
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Strong problem-solving and critical thinking abilities
Valid driver's license and reliable transportation to travel to patients' homes.
