- Manages and/or recommends evident-based medication therapy, including initiation, administration, modification and discontinuation of therapy according to the patient's responses, change in condition or concomitant therapy in order to optimize outcomes.
- Coordinates the timing and collection of drug concentration samples, interprets results, adjusts medication dosages or makes recommendations regarding adjustment and monitors responses to treatment.
- Assists in the communication and coordination of pharmacy activities with Nursing Services by making recommendations for solutions/procedures in applicable areas.
