    Job Title: Outpatient Physical Therapist

    Overview: We are seeking a dedicated Outpatient Physical Therapist to join our team. As a physical therapist, you will provide evidence-based rehabilitation services to patients in an outpatient setting, helping them restore mobility, reduce pain, and improve overall function.

    Responsibilities:
  • Patient Evaluation and Treatment Planning:
    • Conduct thorough initial evaluations to assess patients' functional abilities, movement impairments, and pain levels.
    • Develop individualized treatment plans based on assessment findings, patient goals, and evidence-based practice guidelines.
  • Therapeutic Interventions:
    • Provide hands-on manual therapy techniques, therapeutic exercises, and modalities to address musculoskeletal and neuromuscular conditions.
    • Incorporate functional training and task-specific activities to improve mobility, strength, balance, and coordination.
  • Patient Education and Home Exercise Programs:
    • Educate patients about their conditions, treatment options, and self-management strategies.
    • Prescribe and demonstrate appropriate home exercise programs to promote continued progress and prevent re-injury.
  • Progress Monitoring and Reassessment:
    • Monitor patients' progress throughout the course of treatment, adjusting interventions as needed to optimize outcomes.
    • Conduct periodic reassessments to evaluate treatment effectiveness and modify goals and interventions accordingly.
  • Collaboration and Communication:
    • Collaborate with interdisciplinary team members, including physicians, occupational therapists, and other healthcare professionals, to coordinate care and ensure comprehensive treatment.
    • Communicate effectively with patients, caregivers, and referral sources to provide updates on progress, address concerns, and facilitate continuity of care.
  • Qualifications:
    • Doctor of Physical Therapy (DPT) degree from an accredited physical therapy program.
    • Licensure or eligibility for licensure as a physical therapist in the state of practice.
    • Strong clinical reasoning and manual therapy skills.
    • Proficiency in designing and implementing evidence-based rehabilitation programs.
    • Excellent communication, interpersonal, and documentation skills.
