    Job Type Full-timeDescriptionMental Health Association of Nassau County (MHANC) is a not-for-profit membership organization dedicated to improving mental health in our community through advocacy, education, program development, and the delivery of direct services.

    Our organization is looking for a motivated individual with a calling to help adults with mental illness by becoming a Supportive Housing Counselor.

    Responsible for providing supportive services to individuals in the Supportive Housing Program at MHANC that is consistent with their personal recovery goals, cultural background and beliefs.

    Essential Duties & Responsibilities Participants are to receive weekly visits for the first 4 weeks of admission into the program.

    Conduct face to face contact with the participants, a minimum of once a month or more often as needed. Conduct telehealth contact as needed.

    Provide supportive services which include, but are not limited to, daily living skills, assessment for and linkage to other community services as well as teaching participants how to be appropriate tenants in an apartment setting.

    Assist residents with gathering/completing financial documentation for the purpose of recertification's, leases, etc. Provides crisis management services.

    Coordinate care if participants are hospitalized including but not limited to; hospital visits, consultation with hospital staff, treatment teams, family members, coordination of discharge and post discharge after care services, provide updates to supervisor.

    Assists participants in the development of realistic and measurable personal recovery goals that are culturally sensitive to their beliefs and background as it relates to their recovery.

    Assists in the admissions and discharges of participants in the program, including assisting the resident pack/unpack belongings and process paperwork required for entitlements, etc.

    Act as liaison between participants, landlords and various treatment team members. Conduct fire safety and disaster preparedness drills in a safe and professional manner. Provide oversight for all participants living areas to ensure health and safety are met.
    Conduct medication reviews quarterly or as needed.
    Complete reports as required, using Electronic Medical Records. Ensure that all required documentation is completed in a timely manner.
    Participate in consistent staff meetings including one on one and group supervision.
    Attend meetings/seminars/in-service trainings as required.
    Adhere to the agency's Quality Improvement and Compliance policies and procedures.
    Adhere to all agency policies and procedures applicable to the program.

    Develop and maintain a good working relationship with all staff to assure services and activities are adequately maintained to meet the needs of the participants.

    Other duties as assigned.

    Maintain professional status if applicable through in-service and other trainings and participate in continuing educational programs designed to assure knowledge of changes in profession and/or the mental health field.

    RequirementsEducation & Experience Required
    • Bachelor's Degree in a Human Service field.
    CPR and First AidMinimum of 2 years working with individuals diagnosed with a mental illness. Other Qualifications Requires travel to various locations in Nassau County. Must have valid NYS Driver's License and own transportation.

    Must have working knowledge of MS Word, Basic InternetMust have good written and oral communication skills, Must have the ability to multi-task.

    Must have patience, tact a cheerful disposition and enthusiasm, as well as the willingness to engage challenging participants. Salary Description $19.50 per hour
