    Job Overview
    Mass General Brigham is seeking a skilled Mental Health Case Manager to join our team at Salem Hospital. As a key member of our interdisciplinary treatment team, you will provide inpatient case management services for adult patients struggling with mental illness and co-occurring substance use disorders.

    The ideal candidate will possess a master's degree and relevant experience in case management or a related field. We encourage new graduates to apply, as this role offers opportunities to meet licensure requirements in LMHC or LICSW tracks.

    About the Role:
    • Provide comprehensive psychosocial assessments, formulate individualized treatment plans, and coordinate care with families and outpatient providers
    • Conduct treatment and discharge planning meetings, crisis and safety planning, and arrange aftercare for patients
    • Collaborate with an experienced treatment team to develop and implement effective treatment strategies
    • Work closely with insurance companies to ensure seamless transitions and optimal patient outcomes

    Salary Information:
    We offer a competitive salary range of $ $82,492 - $104,952 per year, based on experience and qualifications. This estimate reflects the minimum job requirements and does not include additional compensation elements such as sign-on bonuses.
