Job Summary
The Transitions of Care Clinical Staff Pharmacist is critical in ensuring patients are prepared to be transitioned to different levels of care and have the information necessary to take their medications safely, appropriately, and consistently.- The pharmacist will review the medications prescribed for appropriate dosing, frequency, and indications and address any discrepancies or recommendations with the patient's primary team or prescriber.
