Hilltop Village Nursing and Rehabilitation provides short- and long-term nursing and rehabilitation services to the senior population in our community. Physical Therapist Job Responsibilities:
· Develop and implement the Physical Therapy plan of care for facility residents and short-term patients.
· Provide supervision, as needed, for staff Physical Therapy Assistants in delivery of the PT plans of care.
· Assist as needed with patient screenings for the need for Physical Therapy.
· Communicate progress and any barriers toward PT goals with interdisciplinary team members.
· Complete timely PT documentation in compliance with regulatory, licensing, and payor requirements.
· Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
· Provide and document patient, family, and caregiver training regarding each PT patient's specific plan of care and recommendations for follow up.
Schedule: Full-Time, Part-Time, and PRN
Education, Experience, and Licensing Requirements:
Current Texas licensure as a Physical Therapist required.
