    RAD Shelter/ Street Outreach


    The function of Street Outreach is to make contact with and coordinate service plans for members of our community experiencing homelessness that are not currently accessing the Emergency Shelter.

    This includes individuals staying at the St.

    Benedict's Encampment and those meeting the criteria of "Literal Homelessness" set forth by the State meaning those living in conditions not suitable for human habitation, on the streets or in vehicles.

    Many of these individuals may have reservations about accessing services provided by other departments within La Puente or other organizations that they may view as institutional in their nature.

    The Emergency Shelter provides a temporary living situation for men, women, and families working on obtaining housing or finding housing more suitable for their current circumstances.

    It also provides multiple hot meals for the community and serves as a hub for accessing other services provided by La Puente as a whole as well as referrals to other community resources.

    Member Duties :

    Homelessness Outreach Case Manager's main function is to create connections with members of the homeless community and assist them in navigating the often complex and difficult processes involved in obtaining and maintaining stable housing.

    This includes but is not limited to learning about and making referrals to outside agencies and other services within La Puente, conducting assessments regarding the vulnerability of homeless individuals, collecting and organizing data that is relevant to those individuals' situations, planning and serving meals at the Shelter and keeping up with the daily tasks of the Shelter.

    Additionally, the member must be comfortable with rotating through being on call for clients after hours which may require basic services such as supply distribution and navigation of various crisis situations.

    The member will be required to assess the needs of the individual in response to the clients requests and find the best options for that person.
