Job summary
We are looking for a compassionate Hospice Licensed Practical Nurse (LPN) to join our team. Reporting directly to the Administrator or Executive Director, you will play a vital role in delivering hands-on hospice care to patients and families in their homes,assisted living, or facility settings.
Your dedication will help provide comfort and support during the most critical moments of life.
- Provide direct nursing care services to hospice patients
- Assist the interdisciplinary team in assessing patient/family needs
- Report any changes in patient conditions promptly
