    Our Lewiston office has a 32 hour position available for a Nurse Practitioner

    Patient Care Duties: The NP may be expected to see patients at the Androscoggin hospice house, in the community, in facilities or hospitals as needed. They must demonstrate expertise in interpersonal and communication skills to foster therapeutic relationships. They will exemplify the agency values of integrity, compassion, excellence in practice, innovation, and community. In keeping with hospice and palliative care best practice, a patient's cultural, spiritual, and emotional needs should be assessed and attended to.

    Clinical Care Responsibilities:

    • Assist family meetings to clarify goals of care, code status and, when appropriate, transition to hospice.
    • Assist with or facilitate family meetings to clarify goals of care, code status and when appropriate, transition to hospice.
    • Coordinate care among both internal and external clinicians and health care providers.
    • Provide on call services via phone for hospice patients, both in the community and at the hospice house.
    • Face to face visits for Medicare beneficiary patients, including documentation regarding life expectancy and hospice eligibility.
    • Participate in care coordination meetings as needed, including mornings ICU rounds, St. Mary's oncology, hospice house and palliative care interdisciplinary team meetings.
    • Act as a resource to staff, patients, family members and attending providers regarding pain and symptom management.
    • For patients at the Hospice house, serve as attending or covering provider and is responsible for ordering medications, treatments, and testing as appropriate.
    • Perform palliative care initial consultations and follow up visits in the community or acute care setting as requested by the primary care team.
    • Provide care based on current clinical practice guidelines and evidence-based practice.
    • Provide expert assistance with advance care planning discussions.

    Professionalism:

    • Knowledge of hospice regulations and standards of care.
    • Strong computer and documentation skills.
    • Knowledge of billing and coding practices to ensure appropriate documentation and full reimbursement for services.
    • Participation in QI programming as needed including development of metrics for outcomes.
    • Assist with development of policies, standards of care and program development as requested.
    • May be involved in workgroup to enhance systems and processes within Androscoggin home health and hospice.
    • Provide education in house and with contracted organizations to enhance understanding of hospice and palliative care.

    Professional Development/Education:

    • Maintain Board certification in hospice and palliative care.
    • Attend national conferences to enhance practice with current medical information, network opportunities with other providers of palliative care.
    • Maintain required education hours to maintain certification.
    • Act as preceptor for NP students and medical students.
    • Participate in educational programs as a lecturer when necessary to professional organizations.
    • Provide education in house and with contracted organizations to enhance understanding of hospice and palliative care.
    • Provide education to public organizations as needed.
    • Provide education to patient and families experiencing life limited conditions.

    EDUCATIONAL REQUIREMENTS: Master's Degree in nursing and certification by the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC) or other recognized national certifying bodies. Licensed by Maine Board of Nursing as an Advanced Practice Registered Nurse (APRN). Certification ACHPN highly encouraged within 2 years of hire if not already attained. Meet the supervision requirements of 24 months supervision by a physician.

    EXPERIENCE: At least two years of practice, additional experience in oncology, hospice and/or palliative care preferred. Preferred certification in hospice and palliative care. Excellent computer skills necessary.

    LICENSURE/CERTIFICATION: Current state APRN licensed; NP certification by ANCC or other nationally recognized certifying body.

    JOB LOCATION: This position will cover all sites. Travel to long-term care facilities, patient homes, hospitals, physician offices, and rural offices may be required on an intermittent basis.- Primary location- Central and Western Maine.
