    YOUR SPECIFIC RESPONSIBILITIES:

    This is an independent professional position that will serve as a perinatal navigator coordinator. This position is responsible for making contact with pregnant women, interconception women and families to conduct an assessment of their needs, determine their eligibility for Family Planning and Prenatal Health, WIC, Healthy Start home visiting services and linkage to supportive services to maximize families' health, well-being, and self-sufficiency. This position is responsible for striving to assure each participant's continued involvement in prenatal and child health care as well as other needed community and Healthy Start services, through actively engaging the participant and building on the families' strengths, assets and goals. This position conducts outreach; provides education in a group setting and plans, coordinates and facilitates health care services for identified high risk participants via telephone calls, home visits, office visits and/or community site visits (i.e. Department of Corrections Facility, UCP/BETA) or a mutually agreed upon location to assist with continuity of care.

    Makes telephone calls and home visits to: pregnant women; parents of at risk infants and women between pregnancies (interconception) who have a loss, either through miscarriage/abortion, stillbirth/fetal death, infant death or have a baby adopted or removed from the home. Conducts assessments of client's total medical and social needs through interview and observation. Establishes rapport and develops relationships with families starting with initial screening and/or contact. Identifies, evaluates and assesses, in collaboration with families, their strengths, resources, needs, and priorities. Facilitates planning and problem solving with participants and families. Addresses identified risks and needs. Provides information, education, and encouragement needed to inform and/or motivate families to take steps necessary to change situations placing them at risk. Promotes self-sufficiency and healthy outcomes through encouragement and motivation, reinforcement of heath care regimen, anticipatory guidance, supporting home safety, enhancing parent-infant interaction, promoting continuation in health care promoting health literacy, and managing behavior concerns. Makes maximum use of community resources through information and referral. Develops and monitors the plan of care to assure that the concerns of families are addressed. Collaborates with other providers to assure continuity and coordination of care and advocates on behalf of the participant, including communicating to the providers, social workers, networking agencies and community the participant's strengths, needs, and feelings. Coordinates service delivery and caseload management in a manner that addresses the immediacy of the participants' needs and identified risks to improve outcomes. Ensures client records are secured and confidentiality is maintained.

    Make face-to-face contact with eligible pregnant clients and/or infants and parenting inmates in the Department of Corrections Facility, UCP/BETA Facility, Covenant House and any additional community partnerships. Conducts an assessment of service needs for client and families. Identifies clients' concerns, needs, priorities, strengths and resources. Evaluates clients' physical and emotional well-being, safety, general appearance, environment and knowledge and attitude about pregnancy/parenting etc. Reviews and compiles health promotion and public health education materials developed for use in training programs and schools. Maintains health promotion and public health education library and teaching equipment.

    Teaches preventive health care and reinforces health care regimen. Provides education and materials to individuals and/or families related to maternal child health (i.e. breastfeeding, nutrition, infant development, parenting skills, smoking cessation, family planning, safe sleep practices and inter-conceptual education). Provisions of education are provided during home visits and/or community site visits at the Department of Corrections Facility/BETA Facility, Covenant House and any additional community partnerships. Education will be provided one-on-one and in group settings. Provides health care information and services as a resource specialist to other agencies and civic, school and professional groups as applicable.

    Accurately codes and documents services, education and other pertinent information in participant's electronic record. Completes and submits all administrative documents. Compiles and reconciles data and other information into reports as required.

    Performs other related duties as requested.

    REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

    Knowledge of: counseling techniques and practices; interviewing techniques; principles and practices of social work, nursing, education or counseling; professional ethics relating to counseling or social work and cultural diversity.

    Ability to: provide counseling or social work services to others; plan, organize and coordinate work assignment; actively listen to others; provide casework services to clients; motivate clients to be independent in their health care and social decision making; and establish; communicate effectively; and maintain effective working relationships with others.

    Knowledge of community services Knowledge of basic computer skills, including keyboarding, WINDOWS, and MS Suite.

    Ability to: communicate effectively verbally and in writing; to access community services; coordinate a team approach to the rehabilitation process; and to establish and maintain effective working relationships with others.

    Knowledge of the theories and concepts of preventive healthcare.

    Ability to communicate effectively.

    Ability to interview clients to identify needs.

    Ability to actively listen to others.

    Ability to provide information and referral services.

    Ability to communicate effectively verbally and in writing.

    Minimum Qualifications:

    • A bachelor's degree from an accredited college or university or higher in social work, social sciences (psychology, human services, biology, sociology) or a health-related field as nursing, health education, health planning or health care administration.
    • One year of verifiable professional experience in public health, community health, maternal child health, health education, or health planning.
    • One year of verifiable professional experience in health education or teaching experience. For example, La Leche League, teen or adult parenting classes, childcare educators, adoption and foster care worker, and child abuse counseling and/or already certified or licensed in a pregnancy/ infant care related specialty (i.e., childbirth education, lactation, doula, nursing, midwife, massage therapist, PNP, PA etc.).
    • Two years of verifiable professional experience conducting interviews or assessments of clients in a Health or Social Services setting.
    • Requires the incumbent to conduct work in the field at various locations throughout Orange County. In the absence of a county vehicle, you must have access to personal transportation.
    • A valid FL driver's license or state identification card OR a valid driver's license from another state (you will be required to get a FL driver license within 30 days of hire).
    • Willing to work before, during, and/or beyond normal work hours or days in the event of an emergency. This may include working in a Special Needs or Red Cross shelter, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural. Emergency duties may also include in-state and/or out-of-state deployment
    Preferred Qualifications:
    • Two years of verifiable professional case management experience working with pregnant and/or parenting teens and/or women, infants and families.
