Organization:
For over 40 years, Life Enrichment Advancing People (LEAP) has been supporting people with developmental, cognitive, and intellectual disabilities to be actively involved in their communities. To this end we provide small group and individualized residential services, adult case management and training services. LEAP is a private nonprofit organization with a mission to empower people with disabilities to lead joyful lives with dignity, independence, and autonomy by providing comprehensive and individualized support. Our fundamental operating principle is to strive for each supported individual's fulfillment of physical wellbeing, self-expression, and experiences of joy.
Role: Case Manager
As a Case Manager with LEAP, you will be an advocate for and work closely with adults with intellectual and developmental disabilities (IDD)/autism, their families and support teams. This position will evaluate strengths, needs, and desires in many areas including medical, social, educational, and vocational. Our case managers also identify, obtain, and monitor services that individuals require to meet their needs and connect them to community resources. This position requires an ability to work independently to meet caseload assignments, deadlines, and organizational goals.
Success Factors for a Case Manager:
The key qualifications to be successful in this position will include a combination of technical knowledge, interpersonal skills, and a strong commitment to the well-being of assigned members. A valid motor vehicle operator's license in the state of Maine and clean driving record is essential. The key soft skills used every day in this position will include a person-centered approach, cultural competence, and a knowledge of resources and services available to people with IDD.
Requirements:Essential Skills and Talents for LEAP Case Workers:
Collaboration and Communication: Effective case management requires collaboration and communication with other service providers, family members and support systems.
Advocacy: Case managers must advocate for their members to ensure that they receive necessary services and support to achieve their goals. It will be important to be familiar with the legal and regulatory requirements related to IDD and use this knowledge to advocate.
Manage Complex Cases: Some members will require a high degree of problem solving and coordination of services. Strong organizational and management skills will be essential to effectively manage these cases.
Empathy and Compassion: These skills will be required to build trusting relationships with members and their families and provide emotional support when needed.
Position Profile:
Full-time, wage/non-exempt role.
No direct reports
Typical Monday-Friday schedule with infrequent early/later meetings to accommodate flexibility needed for some individuals supported.
Minimum Qualifications:
Education:
Bachelor's degree from an accredited four (4) year institution of higher learning. (Degrees in social work, education, psychology, or closely related fields preferred)
Experience:
At least one (1) year of full-time equivalent relevant human services experience.

Work Conditions for Case Manager:
Primary work will be remote/field-based travelling in the assigned regional area to complete caseload requirements.
There are opportunities to perform admin work in a hybrid central-office located in Farmington ME and your home-based remote location for this role.
Physical Requirements for Case Manager:
Typical Work Conditions. Work is performed in a mix of community and residential environments (e.g., service provider locations, single family homes, apartments, etc.), driving between case locations and office based administrative locations. Required to stand, walk, sit; talk to hear, both in person and on telephone; use hands and fingers to handle and/or feel objects or controls; ability for visual and gestural communications; reach with hands and arms. Regularly required to stoop, kneel, bend, crouch, twist torso and climb stairs.
- Physical Manipulation of Objects: Exerting up to 10 pounds of force frequently and/or 40 pounds of force occasionally, to lift, carry, push, pull or otherwise move objects.
- Other Frequently; Work Alone and Drive a Vehicle. Occasionally: Physically escort adult individuals.
