    AdvocarePosition: Care CoordinatorThe Care Coordinator for Care Management functions to support patients engaged with the Care Management Program.

    They collaborate with the Care Manager (RN) and the entire care team to help coordinate services and connect with patients; with the goal of achieving positive health outcomes by providing the patient with the tools necessary to make healthy life-long lifestyle choices and behaviors, close gaps in care and expedite appropriate, cost-effective care.

    The job requires telephonic, electronic, and in-person communication with patients and coordination with all care providers.

    Essential Functions:
    Support individualized patient care planAddress patient risk factors and/or obstacles to care (social determinants of health)

    Facilitate engagement between PCP and patient (appointment setting, documentation)Maintain predetermined patient caseloadRefer patient to and seek advice and sign off from Care Manager as appropriateParticipation in required care coordination case conferencesFind solutions to difficult and/or sensitive problemsProvide inpatient tracking and coordination from ADT feed notificationsWork as patient advocatePerforms miscellaneous job-related duties as assignedClose quality gapsRequired Skills/Abilities:

    Demonstrates professionalism, confidentiality, and diplomacy and can serve a wide range of employees and patients with equity and tactWorks collaboratively with the Care Center, patients, and the Advocare Population Health team to achieve successAbility to establish and maintain effective working relationships with a diverse group of patients and people at all levels of the organizationCommitment to excellence and high standardsDetail oriented with strong follow-up initiativeAbility to collaborate effectively with team membersEffective verbal and written communication skillsVersatility, flexibility and a willingness to work within constantly changing priorities with enthusiasmMust be able to travel to the Home Office and care centers occasionallyEducation and Experience: High School Diploma/GED and 3+ years of relevant healthcare experience in a similar role; or Associate degree in a health-related or science area of study and 1+ years of experience in a similar role; or Bachelors degree in a health-related or science area of study and a minimum of 1 year of experience in a similar roleWork experience in one or more of the following settings: Acute Care, Ambulatory, Specialty, Physician Office, Care Management or Home Health CareLPN, CMA, or MA preferredPlease note this position is posted on behalf of our partner practices.
