    About the Role

    • Our NP will perform comprehensive health assessment:
    1. Obtain history and perform physical examination
    2. Complete review of medicines and related instructions
    3. Review home safety and psychosocial needs < li > Fulfill documentation needs for STAR HCC measures < / li >< li > Obtain document all pertinent information in patient 's charts case management notes same day patient seen ; include medications end life preferences drivers preventable hospital admissions < / li >< li > Instruct patients caregivers family call prior accessing medical care subject life threatening emergencies < / l i >< p > Our NP must maintain patient safety confidentiality . This role requires prescribe/order procedures treatments medications within scope practice develop high level trust intimacy patients families .< br/>
      This work takes place in our offices but some remote days are available ,
