- Conduct Adult/Geriatric Assessments, medical history review, diagnosis, treatment, health education, and documentation according to Matrix guidelines
- Collaborate with healthcare professionals and caregivers
- Work in various settings including Home Visits, Skilled Facility Visits, and Telehealth Visits
- Master's Degree or equivalent experience with NP licensure
- Current RN and NP licensure with prescription authority
- Board certification in a Matrix-approved specialty
- Minimum 1-year experience as a Nurse Practitioner
- Strong computer skills and communication abilities
- Flexibility and adaptability to change
- Valid driver's license, access to a car, and auto insurance
- Travel may be required to reach patients

About the Position:
Matrix providers conduct comprehensive health assessments during visits that can last up to an hour, gaining valuable insight into a member's overall health and well-being. These assessments can help improve care quality and close multiple care gaps with a single visit.

Nurse Practitioner - PRN/Part Time - Home Risk Assessments
Type: PRN/Part Time - Flexible-Nurse Practitioner
PAY: $95.00 or $100.00 per completed Assessment
Location: Multiple community environments in Texarkana, Texas
Hours: Flexible scheduling, including days, evenings, nights, and weekends
Benefits: PRN/Part Time providers are eligible for 401(k) and limited Voluntary Benefits
Mileage Reimbursement plan
