- Coordinate the RAI process, including completion of the MDS and care planning
- Ensure accuracy and completeness of resident assessments
- Collaborate with interdisciplinary team members to develop individualized care plans
- Monitor and evaluate the effectiveness of care plans
- Ensure compliance with Medicare and Medicaid regulations
- Provide education and training to staff on the RAI process and care planning
- All other job duties given by administration
- Current RN license in the state of PA
- Certification in RAC-CT preferred
- Strong clinical assessment skills
- Knowledge of Medicare and Medicaid regulations
- Excellent communication and interpersonal skills
- Ability to work effectively in a team environment
