    Description

    Summary:
    Provides support for the daily activities of the nursing unit and cares for patients, under the direction of nursing and medical staff, by performing the following duties:

    Essential Duties and Responsibilities:
    Include the following, and other duties may be assigned.

    Prepares medical records for new patients and maintains patient medical records according to established standards.

    Records information such as patient's temperature, pulse rate, and blood pressure into patient's medical record. Communicates abnormal findings to the appropriate RN.

    Records all personal care provided in the appropriate location in the medical record. Reports deviations from the norm to the appropriate nurse in a timely manner.

    Assists physician to obtain and retrieve patient information, make appointments/referrals for patients, and arrange for transfers to other facilities.


    Reviews orders entered and records information, such as physicians' orders and instructions, dietary requirements, and medication information in appropriate medical records.

    Insures lab reports, x-ray reports and other pertinent data are printed upon request.

    Enters discharges, transfer status of the patients into the order entry system, unit log book, and appropriate log promptly.

    Serves as second reviewer of verbal and telephone orders entered by the RN.

    Requisitions supplies designated by nursing staff and as needed to maintain PAR levels for the unit.

    Utilizes established customer service behaviors to answer telephones, call lights, and intercom calls. Provides helpful information or relays messages to patients, nursing staff and medical staff, as necessary.

    Directs visitors to patients' rooms and/or other areas of the hospital, providing direct escort assistance if needed.


    Assists with feeding patients requiring help and setting up meal trays so that all food and utensils are easily accessible.

    Provides census information per established protocol.

    Cleans supply room, staff conference room, soiled utility room, and various refrigerator areas and microwave ovens .

    Performs patient nourishment refrigerator temperature check daily and cleans refrigerator weekly.

    Transports patients within facility, or assists patients to walk.

    Prepares records for patients to be transferred to other facilities. Bathes, dresses, and undresses patients. Assists patients with personal grooming.

    Responsible for identifying and labeling patient's personal belongings.


    Drapes patients for examinations and treatments, and remains with patients to perform such duties as holding instruments and adjusting lights.

    Turns and repositions bedfast patients, alone or with assistance, as needed.

    Changes bed linens, runs errands, and answers telephone.

    Performs point of care/waived testing with glucometers. Following established training and competency validation, performs blood draws, PFT's and EKG's as directed.

    Takes and records temperature, blood pressure, pulse and respiration rates, food and fluid intake and output, as directed. Documents observations daily on patient flow sheets and reports any changes, patient needs or requests to the nurse.

    Discontinues intravenous and urinary catheters.

    Cleans all equipment used by a patient at discharge with approved germicidal solution.

    Advocates for patient's physical, mental and social needs with active listening and follows through with requests.
