    Job Title: Hospice Medical Social Worker (LCSW)

    Job Overview:

    We are actively seeking a compassionate and dedicated full time Medical Social Worker (MSW) to join our team. As an MSW, you will play a crucial role in providing essential social work services to patients and their families within their homes. Your expertise in psychosocial support and community resources will contribute significantly to the holistic well-being of our patients.

    Responsibilities:

    1. Psychosocial Assessment:
      • Conduct thorough psychosocial assessments to understand the emotional, social, and financial factors impacting patients' health and recovery.
      • Collaborate with the interdisciplinary team to develop and implement patient-centered care plans.
    2. Counseling and Support:
      • Provide counseling and emotional support to patients and their families, addressing coping strategies and facilitating adjustment to health challenges.
      • Assist in crisis intervention and bereavement support as needed.
    3. Community Resource Coordination:
      • Identify and connect patients with appropriate community resources, support groups, and services to enhance their overall well-being.
      • Advocate for patients to ensure they have access to necessary social services.
    4. Collaboration and Communication:
      • Collaborate with healthcare professionals, including nurses, physicians, and therapists, to address patients' holistic needs.
      • Communicate effectively with patients, families, and the healthcare team to facilitate comprehensive care.
    5. Documentation:
      • Maintain accurate and timely documentation of assessments, interventions, and coordination efforts in compliance with regulatory standards.
      • Ensure meticulous record-keeping in the electronic health records (EHR) system.

    Role Requirements:

    1. Education: Master of Social Work (MSW) degree from an accredited program.
    2. Licensing: Valid and active state licensure as a Clinical Social Worker (LCSW) is required.
    3. Experience: Previous experience in home health or a related healthcare setting is preferred.
    4. Compassion: Demonstrated empathy, sensitivity, and a patient-centered approach to social work.
    5. Communication Skills: Excellent verbal and written communication skills for effective interaction with patients, families, and the healthcare team.
    6. Resourcefulness: Ability to identify and coordinate community resources to meet the psychosocial needs of patients.
    7. Collaboration: Work collaboratively with the interdisciplinary team to ensure comprehensive patient care.
    8. Documentation Skills: Proficiency in maintaining detailed and accurate documentation in the electronic health records (EHR) system.
    9. Ethical Conduct: Adherence to high ethical standards, maintaining patient confidentiality, and respecting autonomy.
    10. Driver's License: Some roles may require a valid driver's license and reliable transportation for home visits.

    Experience:

    • Clinical Assessment: Proficiency in evaluating patient and family needs.
    • End-of-Life Care: Understanding of palliative care principles and practices.
    • Counseling Skills: Experience in grief and bereavement support.
    • Case Management: Coordinating care plans and community resources.
