    POSITION OVERVIEW:

    This role is based in San Rafael, MARIN COUNTY, NORTHERN CA.
    Ritter Center is looking for our next Housing

    Case Manager.

    The Housing Case Manager for the Housing and Disability Advocacy Program (HDAP) works directly with a housing team to identify and house up to 20 people with previous experience of chronic homelessness.

    This role will also work closely with other organizations to document the support those case managers are providing to their existing 14 HDAP clients.

    The goal of this grant is to identify and support an additional 6 HDAP clients with this finite amount of funding.

    This position is a time-delimited role from date of hire through June 2025.

    Job Duties and Responsibilities:


    Conduct outreach in the community, with partner organizations, and with the county of Marin to identify potential referrals for the HDAP program.

    Collaborate with Social Service agencies to ensure appropriate support for clients in the process of applying for SSI, advocate for clients as needed, follow up with clients and other agencies regarding the transition of clients into other housing programs and services, and record all outcomes.

    Perform ongoing monthly home visits, at least twice a month, to all assigned tenants in the agency's housing programs and record data from each visit.

    Actively participate in Ritter's BWELL team meeting to update treatment plans for HDAP and Housing First clients.

    Assess client needs and work with all staff to provide a seamless, effective service plan; make appropriate referrals to other service providers when necessary.

    Assist clients in developing independent living skills and setting realistic goals; provide the support needed to access agencies (bus tickets, identification cards, etc.).

    Maintain accurate records of referrals and client advocacy
    Attend assigned case management meetings
    Meet with supervisor and attend staff meetings and trainings as scheduled.
    Adhere to the goals, objectives, and personnel policies of Ritter Center

    Perform other duties as assigned, which may include performing Representative Payee Duties (i.e., budgets, check requests) and maintaining Payee Program records.


    QUALIFICATIONS & REQUIRED SKILLS:
    Bilingual Spanish speaking skills are not required but preferred.
    Per our Federally Qualified Healthcare Center (FQHC) requirements, employment at Ritter Center requires compliance with being able to produce a negative TB test prior to the start of employment, as well as being

    fully vaccinated against COVID-19

    (unless applying for an exemption) per the August 5th, 2021 State Public Order for Healthcare Facilities, providers and employees.
    Undergraduate degree from an accredited institution
    Good skills in organization and ability to multi-task and remain calm
    Experience working in social service setting; ability to be empathic and compassionate with clients combined within a framework of appropriate boundaries
    Understanding of the needs of low-income working people and people who are homeless
    Strong people management and communication skills
    Need to possess a valid and clean CA driver's license
    Knowledge of Ritter Center services, policies and procedures, and community
    Basic computer knowledge is required, and accuracy in data entry
    Must possess a valid California class C driver's license and have a satisfactory driving record
