    Description

    We are looking for a Full-Time Home Health Physical Therapist

    The Physical Therapist in Home Health evaluates patient care needs related to functional status, mobility, endurance, pain, and medical conditions. Based on the assessment, the Physical Therapist develops a treatment plan, performs interventions to enhance the patient's well-being, and monitors progress.

    • Evaluates the patient's functional status and body systems, consulting with physicians to develop a therapy plan.
    • Provides services within state practice laws, following the plan of care and collaborating with the healthcare team.
    • Observes and reports patient responses and changes to supervisors.
    • Treats patients to relieve pain, restore function, and maintain maximum functioning.
    • Provides education to healthcare members, patients, and families on the plan of care and progress.
    • Visits patients within the service area and nearby agencies as required.
    • Completes progress notes and evaluations in a timely manner.
    • Participates in staff conferences and committees.
    • May supervise home health aides and PTA as per regulations.
    • Participates in weekend rotations for services.
    • Prepares discharge and summary reports regularly.
    • Provides in-service education as needed.
    • Adheres to Therapy Standards of Practice and maintains licensure.
    • Engages in ongoing education for professional development.
    • Performs any other assigned duties.

    Licensure Requirements

    • Current FL Physical Therapy licensure
    • Current CPR certification
    • Must have a valid driver's license, vehicle insurance, and access to a reliable vehicle
