- Consult with the Program Director to develop client Individual Service Plans (ISP), providing medical information that highlights the clients' medical needs.
- Develop client Health and Safety Plans, and collaborate with the team on the implementation of health care procedures for each identified health need.
- Assist the Supported Living Facilitator (SLF) in reviewing client ISPs each month to ensure the ISP reflects the clients' changing needs.
- Provide specialized and client specific training for Direct Service Providers (DSP).
- Provide direct service and support to clients. Perform delegated independent nursing functions using established procedures, policies, guidelines and standards.
- Other duties as assigned.

On My Own Independent Living Services is expanding our Supported Living Services (SLS) program to include the support of a Licensed Vocational Nurse (LVN).
This role will start as part-time and offer the opportunity to be on the ground floor as we implement these new enhanced SLS services. This role may expand beyond part-time in the future as we work with other programs to get more of our clients access to these services.
