    Job Title:
    Licensed Vocational Nurse - Skilled Nursing Facility

    Overview:

    We are seeking a compassionate and dedicated Licensed Vocational Nurse to join our team at [Skilled Nursing Facility Name].

    The LVN will play a pivotal role in providing exceptional care to our residents, ensuring their well-being and comfort within our skilled nursing environment.


    Responsibilities:

    Patient Care:

    • Administer nursing care to residents based on individualized care plans and physician orders.
    • Perform and document routine assessments, monitor vital signs, and evaluate resident health status regularly.

    Medication Administration and Treatment:

    • Administer medications and treatments as prescribed, adhering strictly to medication administration protocols.
    • Assist in the development and implementation of care plans in collaboration with the healthcare team.

    Wound Care and Monitoring:

    • Provide specialized wound care as required, ensuring proper documentation and follow-up.
    • Monitor and assess changes in resident conditions, promptly reporting any concerns to the registered nurse or physician.

    Patient and Family Education:

    • Educate residents and their families about their conditions, treatment plans, and self-care strategies.
    • Offer emotional support and guidance to residents and their families throughout their stay at the facility.

    Documentation and Compliance:

    • Maintain accurate and up-to-date medical records and documentation in compliance with facility policies and regulatory standards.
    • Participate in quality assurance activities and assist in ensuring compliance with state and federal regulations.

    Qualifications:


    • Active LVN license in Texas.
    • Previous experience in a skilled nursing facility or long-term care setting is preferred.
    • Proficient in nursing assessments, medication administration, and wound care.
    • Strong communication and interpersonal skills.
    • Ability to work collaboratively in a team-oriented environment.

    Preferred Qualifications:

    • Familiarity with electronic health records (EHR) systems.
    • Additional certifications related to geriatric nursing or long-term care.
