    Key Duties:

    • Screen and evaluate patients with cognitive and/or ADL physical functioning disorders
    • Develop appropriate care plans following regulatory and clinical practice standards
    • Provide skilled treatment interventions based on evidence-based practice
    • Adjust treatments based on patient progress and evaluation outcomes
    • Offer consultation and counseling to patients, families, caregivers, and service providers
    • Communicate effectively with the health team regarding patient progress and treatment plans
    • Provide comprehensive discharge summaries per regulatory and clinical requirements

    Qualifications:

    Licensed Occupational Therapist
