Job summary

We are seeking an experienced Occupational Therapist (OT) to join our dedicated Home Health team and provide exceptional care throughout the region.

Responsibilities

  • Provide, coordinate and oversee skilled intermittent care to patients and include instruction and education of patients and/or caregivers to promote maximal functional status, in the patient's home.
  • Administer treatments, monitor patient conditions, and report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or as needed Director of Nursing.
