    The Coordinated Diversion Oregon State Hospital (OSH) Liaison - Mental Health Consultant (MHC) position evaluates, investigates, determines, and reviews extended care coordination needs for clients with specialized mental health care needs who are legally/criminally exposed and who are currently residing at the Oregon State Hospital under .370 orders.

    The OSH Liaison - MHC promotes the availability and quality of individualized community-based services and supports, primarily housing.

    The position is integral to conduct clinical consultation and assessments to facilitate service coordination for individuals with Coordinated Diversion enrollment with serious mental health problems to limit institutionalized care and yet live in the least restrictive setting possible.

    The OSH Liaison - MHC evaluates and determines the best specialty services and culturally appropriate available resources with community providers, to meet the client treatment plan.

    The OSH Liaison - MHC works remotely from home and serves in the community.


    As a newly hired Mental Health Consultant, you will receive a one-time New Hire Recruitment and Retention Incentive payment of up to $1,500 on your first paycheck.

    The incentive will be prorated based on the position's FTE status.

    As the OSH Liaison - MHC , other duties will include:

    Case Coordination, Consultations, Court Attendance, and Meeting Attendance


    • Intensive care and culturally responsive case coordination, direct consultation, oversee, evaluate and coordinate behavioral health services performed by internal and community providers, and conducts evaluations for dually-enrolled Forensic Diversion and Choice eligible clients with complex needs including clients with specialized mental health care needs to ensure that client needs are appropriately and adequately addressed by service providers.
    • The client specialized health-care needs addressed may include complex medical needs, high health care needs, housing needs, special mental and physical healthcare needs and co-morbidities, multiple chronic conditions, behavioral health issues including chemical dependency, and those identified as requiring care coordination and specialized resources.
    • External partners may be discharge planners from state hospitals, residential treatment providers, Court systems, case managers, substance use disorder treatment providers, acute care settings, community corrections, police, and other mental health providers.
    • Initiate, attend, and participate in regularly scheduled and urgent/impromptu meetings to collaboratively problem solve any barriers to services/discharge planning.
    • Locate and assist with arranging specialty mental health services, as appropriate, upon request by client, physician or provider, to ensure that client needs are appropriately and adequately addressed.
    • Exchange risk assessments for clients with physician or provider to increase the knowledge of overall risk of specific clients who have been identified with a mental health diagnosis to determine if specialty service is appropriate under the client's treatment plan.
    • Review authorization patterns for capacity and length of stay for housing or other needs
    Documentation and Communication


    • Document all coordination efforts in the electronic health record, including, but not limited to, communication with service/placement providers, member/provider contacts and meeting attendance/staffing.
    • Complete assessments based on clinical information to provide clinical input on level of need/placement, and gaps in care.
    Charting and Records


    • Track client information and treatment needs, internal and external partner/provider decisions and recommendations, etc., to report to managed or coordinated care organizations for funding or charge-back actions, grant requirements, etc.
    • Provide clinical recommendations to community partners regarding needs and recommendations for services needed.
    • Collect medical, mental health, and social information from treatment records and medical charts, make clinical observations and present findings and recommendations at internal and external stakeholder meetings.

    Minimum Qualifications (REQUIRED):

    Education/Certification:

    Must have a Masters degree and meet the standards of the Qualified Mental Health Professional - (QMHP-R) with the Mental Health & Addiction Certification Board of Oregon (MHACBO).

    Please visit the MHACBO website to confirm your eligibility to apply. This position requires a QMHP - C within six (6) month of hire, and/or licensed Board of Oregon (LPC, LCSW, PHD,etc)


    • Masters degree in psychology or
    • Masters degree in social work or counseling or
    • Masters degree in a behavioral science field

    Preferred Qualifications/ Transferable Skills*:
    You do not need to have the following preferred qualifications/ transferable skills to qualify. However, keep in mind we may consider some or all of the following when identifying the most qualified candidates.


    • Appropriate Oregon Licensing Board LCSW, LCP, Clinical Psychologist (preferred)
    • Screening and/or Investigator experience in an adult and/or child protective services program.
    • Knowledge of challenges and barriers faced by those with Severe and Persistent Mental Illness (SPMI) and the systems they encounter in Residential Services, Adult Protective Services and Crisis Services.
    • Lead with race through actions and advocacy with internal programs, with community partners and across departments throughout the county.
    • Demonstrate the ability to flex style when faced with myriad dimensions of culture in order to be effective across cultural context.

    Salary/Pay:

    However, actual compensation will be determined in accordance with the Oregon Equal Pay Law and will take into account factors such as the candidate's relevant experience and education.
