    Overview:

    The mission of NorthPoint Health & Wellness Center's (NorthPoint) is to create a healthier community. Three pillars provide the foundation for serving the North Minneapolis community: Whole-Person Integrated Care, Building Community Well-Being and Health Equity.


    Whole-Person Integrated Care

    NorthPoint offers a continuum of social and health services that integrate medical, dental, and behavioral health with human services to improve quality of life. We strive to serve our community by addressing the physical, behavioral, social, and spiritual aspects of health in culturally responsive and trauma-informed ways.


    Building Community Well-Being

    The purpose of this role is to oversee the day-to-day operations of NorthPoint's Community Outreach Department, including a Food Shelf that serves approximately 2,000 families per month. The Manager of Community Outreach manages, develops, implements, and monitors and evaluates services and programming that support families in living healthier lives by resolving human, social, and physical needs. In the pursuit of health equity, this position will promote healthy lifestyle choices, improve health disparities, organize events that benefit and enhance NorthPoint's mission of partnering to create a healthier community, ensure food and other basic needs are met for individuals and families, and reduce barriers to services and programs. The Manager will work in collaboration with all areas of the organization to integrate care across health and human services programs, with a focus on food insecurity and healthcare access.

    Minimum Qualifications:

    • Bachelor's degree in Public Health, Human Services, or a related field and five (5) years of related experience working in a community health and human services environment. Associates degree in Public Health, Human Services, or a related field and seven (7) years of related experience working in a community health and human services environment. High School Diploma or GED with ten (10) years of related experience working in a community health and human services environment.
    • Three (3) years of experience supervising a diverse team within a social service or related environment.
    • Three (3) years of experience managing grants, projects, and/or programs related to food insecurity, healthcare, public health, and/or social determinants of health initiatives.
    • Two (2) years of experience implementing culturally based program and service responses to individuals from marginalized communities.
    • Proven record of accomplishment of designing and executing successful program design, implementation, and meeting outcome metrics.
    • Ability to form and maintain internal and external partnerships that support the organizational mission, values, and goals.
    • Demonstrated knowledge and ability to use internal data and research from other sources to formulate program design and program improvement strategies for the delivery of customer focused integrative human services.
    • Excellent verbal and written communication skills with a high-level attention to detail, maintaining clear and effective dialog with all levels of employees, guests, and the community.
    • Advanced PC skills in Microsoft Office, including Word, Excel, PowerPoint, Teams as well as proficiency with various technology including mobile devices.
