    StarCare Specialty Health System (StarCare) is seeking a full time Clinic Care Coordinator Lead to join its outstanding Outpatient Clinic supporting people with mental illness at Sunrise Canyon.

    Responsibilities:
    • Coordinate and oversee activities of the clinic and clinical team members to ensure compassionate, effective, and efficient delivery of high-quality clinical care for patients.
    Essential Duties: (To accomplish this job successfully, an individual must be able to perform each essential function satisfactorily)
    • Coordinate and supervise the activities of Care Coordination Team members and other clinic staff engaged in patient care.
    • Organize and assign duties and tasks to Care Coordinators.
    • Establish Care Coordination work schedules monthly.
    • Monitor daily performance of care coordination and clinic to ensure duties are completed accurately, efficiently, and timely.
    • Assist with interviews. Conduct orientation and training on clinical policies and procedures.
    • Identify and recommend solutions to Clinic Director and members of the outpatient clinic team
    • Ensure high quality clinical care for patients by reviewing and researching concerns or complaints.
    • Assist medical assistants, nurses, and providers with maintaining patient flow to ensure patients are seen timely and efficiently.
    • Assist with or conduct periodic staff meetings with care coordination.
    • Maintain excellent communication and effective working relationships with patients, providers, clinical and clerical team continuity of high-quality patient care.
    • Perform all other duties as assigned.
    Schedule: This is a full-time/exempt position scheduled Monday - Friday, 8am - 5pm, with flexibility to meet the needs of the department.

    Qualifications

    Qualifications:
    • Bachelor's degree in healthcare administration or human services field.
    • 2 years of experience at a healthcare facility in a clinical coordinator role (essential).

    • Meticulous attention to detail with the ability to multi-task.
    • Strong organizational, administrative, and planning skills.
    • Ability to work under pressure and react effectively to emergency situations.

    Additional Requirements:
    • Must pass a pre-hire drug screen and criminal background check.
    • Must have a Texas driver's license and be insurable under the Agency's insurance.
