    Social Services Director (Social Worker)

    The Job

    We seek a highly skilled Social Services Director (Social Worker) to support the successful execution of our mission and core values by improving or maintaining each resident's ability to control everyday physical, mental, emotional, spiritual, and psycho-social needs.

    Responsibilities

    • Develops and implements social services programs to meet the needs of residents.
    • Conducts social history assessments and develops rapport with residents and families.
    • Explains and obtains Advanced Directives from residents and families.
    • Collaborates with nursing staff to ensure accurate Do Not Resuscitate orders are obtained for residents.
    • Develops policies and procedures to identify medically-related social and emotional needs of residents.
    • Applies social work theory and knowledge to restore or enhance social, psycho-social, or biopsychosocial functioning of residents.
    • Participates in Interdisciplinary Care Plan Meetings and provides input on patient psycho-social needs.
    • Provides information on Medicare/Medicaid and other financial assistance programs available to residents.
    • Offers direct services to residents and families, including counseling and support.
    • Completes required documentation, including social history, social assessment, and reassessments.
    • Participates in community planning related to the interests of the facility and services needed by residents and families.
    • Clients residents' social service agencies and schedules medical appointments as required.
    • Makes recommendations to the Administrator concerning social services operations.
    • Contributes to quality assessment and improvement of social services programs.
    • Stays current on federal and state regulations, professional standards, and makes recommendations for policy changes.
    • Notifies families and/or patients of Care Plan meetings and documents accordingly.

    Requirements

    • Bachelor's or Master's degree in Social Work, with licensure in the State of Texas.
    • Minimum two years of social work experience in a supervisory capacity in a hospital, skilled nursing care facility, or related medical facility.
    • Outstanding organizational skills and ability to deal tactfully with associates, patients, family members, visitors, government agencies/personnel, and the general public.
    • Proficiency in computer systems (Word, Excel, Electronic Health Record system, etc.).
    • Good standing with Federal and State Government.
    • Passion for working in a retirement community with the geriatric population.
