    Under the direction of the Chief Nursing Officer (CNO), this position is responsible for acting as a team lead within the referrals department. This position includes receiving, processing, scheduling and following up on all referral requests, including in-house and outside referrals for diagnostic testing, medical specialists, or other providers. This position provides timely, courteous, friendly, and confidential assistance to all patients and ensures that they have a positive and successful experience in the referral process. Promotes team work within the office environment.

    Duties:

    • Acts as system navigator and lead for the referral department staff and providers within the organization.
    • Reviews referral ques, staff follow through, and designated timelines to ensure goals and quality standards are being met within the department.
    • Establishes and maintains relationships with identified service providers and community agencies to aid in the referral process.
    • Assists CNO in collecting data for analysis, tracking and reporting of program activities
    • Establishes patient eligibility, initiates patient enrollment, and processes referral for patients to outside agencies according to Morton Comprehensive Health Services' policies, procedures, and responsibilities.
    • Ensures the timely processing of referrals and feedback to the client and provider, in accordance with Morton's Referral Management: Coordination of Care Policy.
    • Coordinates with physician's office personnel and patients to obtain accurate registration information.
    • Answers patient inquiries and takes messages for follow-up.
    • Verifies and enters patient's demographic data into the computer database.
    • Schedules patients for appointments to include laboratory tests, medical examinations, radiologic services and consultations.
    • Relays information to patients regarding appointment(s).
    • Attends referral networks and update courses as needed.
    • Participates in accreditation and quality improvement activities.
    • Maintains confidentiality within the workplace and within the community.
    • Handles protected health information in a manner consistent with Health Insurance Portability and Accountability Act (HIPAA).
    • Adheres to the Mission and Values of Morton Comprehensive Health Services.
    • Performs other duties as assigned

    Job Qualifications:

    • High School Diploma or GED required.
    • Associates Degree or Career Technical School preferred.
    • Four to five (4-5) years of experience working in medical referrals required.
    • Must be proficient in Microsoft Suite applications and Internet usage.
    • Bilingual in English and Spanish is preferred.
    • Ability to work with patients and staff professionally and tactfully. Position may be required to occasionally deal with hostile persons.
    • Ability to work independently as well as with a professional team.
    • Appropriate manner, conduct, and grooming required.
    • Knowledge of ability to prepare reports utilizing statistical data required.
    • Assumes responsibility for own continued personal and professional growth.
