    The DSP plays a vital role in helping members achieve their goals by offering assistance and interventions that promote independence, growth, and family support.

    Responsibilities:

    • Embracing Diversity: Cultivate an understanding and appreciation for diversity, ensuring services are tailored to the cultural, social, and individual contexts of each child/youth and their caregiver/family.
    • Support and Resources: Provide educational, advocacy, and support resources, adapted to the unique circumstances of each child/youth and caregiver/family.
    • Integration and Support: Enhance the child's integration into the caregiver/family unit, bolstering their capacity to support them in home and community settings.
    • Empowering Caregivers: Empower caregivers to independently access and leverage resources, promoting long-term self-sufficiency.
    • Addressing Concerns: Address any caregiver/family concerns related to supporting their child, respecting their cultural and individual contexts.
    • Education and Awareness: Offer comprehensive instruction to deepen understanding of the child's condition or illness.
    • Community Engagement: Engage with families/caregivers and children/youth to promote a holistic approach to accessing community services and activities.
    • Service Plan Updates: Regularly update service plans, obtaining medical consent signatures and ensuring plans reflect members' evolving needs.
    • Consent and Documentation: Acquire necessary consents and documents for newly enrolled members, adhering to all documentation requirements.
    • Communication and Collaboration: Attend monthly waiver service provider meetings, report incidents promptly to the supervisor, and collaborate openly with other service providers.

    Flexibility and Adaptability:

    • Services can be rendered both in the home of the youth or in a community setting, emphasizing a holistic, integrated approach.
    • Must be willing to work four to six hour shifts during after-school hours, weekends and non-school days; number of hours per week is dependent on the client's needs and approval.

    Requirements:

    • Qualifications and Education: Associates Degree minimum; Bachelor's Degree preferred.
    • Preferred Skills:
      • Bilingual Preferred.
      • At least 1 year in a related Human Services role.
      • Experience with the adolescent/special needs demographic.
      • Knowledge and experience working with children facing mental and behavioral health challenges.
      • Strong cognitive skills tailored to the position.
      • Professional demeanor with an approachable, friendly attitude.
      • Comprehensive documentation skills, ensuring clarity, accuracy, and timeliness.
