    Description

    Job Title:

    Provides direct and indirect services for individuals involved in the Criminal Justice System who are diagnosed with a mental illness and/or co-occurring mental illness and substance use disorder.

    Duties and Responsibilities:

    1. Assesses client needs and implements services accordingly, ensuring a safety net of resources. Communicates clearly with clients about system expectations and provides periodic assessment and cross-system planning to meet their needs.
      • Coordinates service planning meetings and provides follow-up support.
      • Cross-system coordination and monitoring of service delivery.
    2. Provides support and advocacy through court proceedings.
    3. Manages caseload effectively, maintaining records and adhering to productivity standards.
    4. Develops and implements service plans, including assessing function level for community placement.
    5. Collaborates with treatment and court partners to determine case management, housing, and treatment services.
    6. Participates in multi-disciplinary approaches with various service providers.
    7. Monitors service delivery and progress with provider agencies.
    8. Engages in ongoing training in mental health and criminal justice fields.

    Essential Requirements:

    1. Act 33 and 34 Clearances.
    2. Institutional Clearances through NCIC.
    3. Pennsylvania motor vehicle license and maximum auto insurance.
    4. Computer skills.
    5. Ability to work independently.

    Qualifications:

    Master's Degree with one year experience working directly with behavioral health needs or criminal justice system, or Bachelor's Degree with two years' experience in the same field.
