- Provide social support navigation for SDOH such as food and housing insecurity
- Collaborate with primary care team members including nurses and providers
- Conduct social needs screening and assessments for referred patients
- Coordinate social services resources like housing, clothing, and mental health services
- Work with hospitals on discharge planning and follow-up care
- Assist in obtaining patient records and necessary medical equipment
- Track care plans and follow-up with patients
- Identify and assist patients lost to follow-up or overdue for care
- Help with specialty referrals and imaging appointments
- Support patients in accessing low-cost prescription options
- Assist with patient assistance program applications
- Participate in regional office and clinical meetings
- Attend provider and community meetings to promote health services
- Identify and address barriers to patient care
- Complete onboarding and orientation process
- Participate in various meetings as requested
- Attend community events to establish relationships with social service agencies
- Document patient interactions and provide necessary updates
- Provide patient education when required
