    Under the general supervision of the Manager and direct supervision of the Supervisor, the Community Health Worker (CHW) will be responsible for supporting Members in improving their health and well-being, through outreach and engagement activities, which are primarily field based.

    The CHW works closely and collaboratively with the care management program team, as well as with the designated medical teams, to ensure high quality and seamless care for Members.

    This position may support multiple units with the Health Services Division.

    Education & Experience


    • Two (2) years of experience as a CHW, Promotora, or Health Navigator, or two (2) years of experience working in community outreach, customer service, or within a medical office, or a Behavioral Health or Substance Use Disorder program required
    • Applicable to Bilingual job code only -Bilingual (English/Spanish) - written and verbal skills required
    • Successful completion of an approved Community Health Worker program or will complete within six (6) months of hire
    Key Qualifications


    • Highly skilled interpersonally with excellent teamwork and relationship skills. Highly skilled in interpersonal communication, including resolving conflict.
    • For Integrated Care Transitional Care:
    • Demonstrate computer skills in application of the platforms, such as Care Director, Care Prominence, and other platforms used by IEHP
