- Conduct assessments, medical history reviews, diagnosis and treatment, health education, referrals, and documentation according to Matrix guidelines
- Collaborate with healthcare professionals and caregivers
- Work in various settings like home visits, skilled facilities, and telehealth
- Master's Degree or equivalent experience
- Current RN and NP or PA licensure
- Board certification in a relevant specialty
- BLS, ACLS, or CPR certification
- Minimum of 1 year experience as a Nurse Practitioner/Physician Assistant
- Strong computer skills and EMR familiarity
- Excellent communication skills
- Positive attitude and team player
- Valid driver's license and access to a reliable vehicle
- Travel to patient locations

About the Position:
During a visit lasting up to one hour, Matrix providers assess a member's health, medical history, medication adherence, and social environment. This in-depth assessment allows us to improve care quality and address multiple gaps in care with a single visit.

Type: PRN/Part Time - Flexible-Nurse Practitioner
PAY: $110.00 per completed Assessment
Location: Various settings in Saint Peter, MN
Hours: Flexible schedule including days, evenings, nights, and weekends
Benefits: Limited benefits for PRN/Part Time providers
Mileage reimbursement plan
