The Community Liaison serves as the face of our Home Health agency within the local market. This individual is responsible for driving census growth by building and maintaining strong relationships with referral sources, ensuring a seamless experience for partners and patients alike.

Responsibilities

    Maintain detailed records of all meetings, activities, and referral trends in the company CRM; review performance data regularly with leadership
  • Collaborate with market leadership to develop tailored growth plans based on data insights and local market dynamics
  • Analyze payer mix and competitive landscape to identify opportunities aligned with Alpine Care's strengths/li>Serve as a consistent point of contact for referral sources, providing timely updates on patient status and care coordination
