    Job DescriptionA Registered Nurse who assists in providing nursing services, education to staff and clients and is a vital member of the Adult Homes Supported Housing team.

    The Registered Nurse is part of the interdisciplinary team and serves as the expert in for the assessment, identification of needs and coordination of care and services to facilitate the patient's progression through the continuum of care in a timely and cost-effective manner.

    Care management including care planning, coordination, and assessment of health, environmental and psychosocial status members on an ongoing basis.
    Additional RequirementsMinimum of two years of experience in specialty or area of practice, e.g. acute care, ambulatory care, long term care, behavioral health. Some lifting of medical equipment and supplies. Prolong hours of sitting and/or standing. Travel to and from clients' home and walking up and down stairs. Ability to travel to clients home and conduct nursing and behavioral clinical visits. Able to carry medications to clients home and back to office. Ability to perform in a busy, changing, multi-tasking work environment. Excellent written and verbal communication skills. Excellent interpersonal skills. Excellent organizational and documentation skills. Proficient in Microsoft applications.


    Education Requirements:
    NYS RN License required; Associates or Bachelor degree required; Master degree preferred Valid driver's license in good standingProficient in Microsoft Office Suite (Outlook, WORD, Excel, PowerPoint) and database navigation required
