    Description

    Job Summary:

    Service Coordinators will work under the direction of a Program Manager to maintain relationships with clients, coordinate case management, collect and analyze data, and contribute to program development and evaluation.

    Schedule:

    Tuesday-Saturday (variable hours)

    Key Responsibilities:

    1. Caseload Management: Maintain a caseload of youth aged 10-17, referred through the Chicago Police Department, ensuring high-quality client services. Perform assessments to determine client strengths and needs, and support goal setting and achievement.
    2. Supervision: Participate in regular one-on-one supervision and team case-planning meetings, as well as group supervisions and training as necessary.
    3. Community Relationships: Develop and maintain positive relationships with coworkers, partner agencies, and referral sources in the community to benefit clients and the organization.
    4. Recreational and Restorative Justice Activities: Assist in planning and executing community events that benefit clients, their families, and communities.
    5. Court Attendance: Attend court sessions with clients as needed, providing the Court with case plans or updates as necessary.
    6. Transportation: Transport youth as required.
    7. Confidentiality: Maintain confidentiality of client information, with full access to client data.
    8. Quality Improvement: Contribute to ongoing continuous improvement efforts within the organization.
    9. Diversity and Cultural Awareness: Effectively work with diverse communities and stakeholders, maintaining cultural competencies and humility.

    Requirements:

    • Bachelor's degree required
    • Three years' experience in case management
    • Effective communication skills

    Preferred Qualifications:

    • Experience working with youth aged 10-17
    • Community experience
    • Willingness to work with the Chicago Police Department
