- Conduct behavioral assessments to identify each member's knowledge, skills, and behavioral requirements.
- Coordinate tailored health coaching to address identified objectives and goals.
- Implement coaching plans using behavior change principles to address member barriers.
- Provide feedback on member goal achievement and clinical issues as necessary.
- Monitor and evaluate interventions, making modifications when needed.
- Develop and deliver wellness promotion programs through group activities, webinars, educational materials, and challenges.
- Bachelor's degree in a relevant field such as Health Education, Exercise Physiology, Respiratory Therapy, or Dietetics, coupled with at least 3 years of experience in health education, exercise instruction, or patient education; or an equivalent combination of education and experience.
- Mandatory bilingual proficiency in Spanish.
- Possession of necessary certifications, advanced certifications, and/or licenses as per specialization requirements.
- Previous experience in health coaching, disease management, and familiarity with behavioral and clinical assessment techniques.
- Understanding of disease management principles.
