Job summary
The RCSU Case Manager provides short-term, intensive case management services to individuals admitted to the Residential Crisis Stabilization Unit.This role focuses on rapid assessment, service coordination, discharge planning, and linkage to community resources to support stabilization and successful transition to a lower level of care.
The Case Manager works as part of a multidisciplinary crisis team in a fast-paced, high-acuity environment.
