Responsibilities
- • Work with clients providing ACT services assume primary responsibility for developing writing implementing evaluating revising overall recovery goals plans• Facilitate development of comprehensive assessments individualized treatment plans substance abuse treatment other tools necessary promote recovery• Provide leadership team assessing planning treating substance abuse assigned clients individually groups facilitate co-occurring evidenced-based practices need identified•2nd list item‣rd list item
- facilitate admissions inpatient CD treatment linkage community resources outpatient support Work clients all aspects psychiatric rehabilitation support psychotherapeutic interventions identified treatment plan include Medication Assistance Support coaching education independent living skills cooking shopping cleaning personal hygiene transportation budgeting organization housing Collaborate schools vocational services encourage competitive employment educational success collaborate youth ACT nurse healthcare providers promote client physical dental wellness goals Attend commitment court hearings required complete required court reports testify when requested File reports Vulnerable Adult Child Protection Ombudsman Office mandated/required reports applicable Work multidisciplinary team consisting psychiatrist team leader mental health professionals practitioners certified peer specialist nurses vocational specialist Co-Occurring Disorder Specialist team coordinator Actively engage clients family members significant others landlords employers client approval Provide on-call crisis intervention covering evening hours serve backup evening weekend staff rotating schedule Available work weekend evenings holidays needed Attend participate scheduled team meetings Provide transportation applicable clients treatment plan goals Promptly respond email text telephone communication Complete documentation reports charting contemporaneously organized manner ACT policy procedure manual Participate actively ongoing professional growth development maintain appropriate professional behavior participate supervision determined supervisor Actively coordinate discharge planning IRTS hospital treatment community services Maintain pattern regular work hours Other duties assigned
