*About the role*

Evergreen Home Health is hiring a *PRN Physical Therapist* to provide in-home evaluations and follow-up visits. You choose *any two days per week*. We guarantee *$500 per day*, typically *4-5 patient visits/day*. Ideal for PTs seeking flexible supplemental income with a supportive, responsive team.

*What you'll do*

* Perform home-based PT evaluations; develop individualized plans of care.
* Deliver skilled therapy to improve mobility, safety, and independence.
* Educate patients/caregivers on exercises, equipment, and home modifications.
* Collaborate with the interdisciplinary team to coordinate care.
* Complete timely, compliant documentation in our user-friendly EMR.

*Qualifications*

* PRN / Per-Diem, *2 days per week* (days of your choice)
