    Description

    Vetted is seeking a RN - Case Management for a travel job in Diamond Bar, California.

    Must have 2+ years of experience.

    This contract pays approximately $15,343/month gross.

    Assignment details:

    • Contract length: 13 weeks
    • Shift length: 12 hour shifts
    • Hours per week: 36
    • Start Date: ASAP

    Job responsibilities:

    • Support care coordination activities across providers to promote continuity of care.
    • Educate patients on treatment plans, follow-up instructions, and self-management strategies.
    • Work 36 hours per week with 12-hour shifts.
    • Perform medication reconciliation activities as directed and communicate discrepancies.
    • Administer ordered therapies and monitor for adverse reactions or changes in condition.
