    Job Title: MDS Coordinator

    Overview: We are seeking a highly skilled and motivated MDS Coordinator to join our team. As an integral member of our healthcare facility, you will play a crucial role in ensuring accurate and timely completion of Minimum Data Set (MDS) assessments, as well as coordinating care plans for our residents.     Responsibilities:
    • Conduct comprehensive assessments of residents' physical, emotional, and psychosocial needs, ensuring compliance with regulatory requirements.
    • Collaborate with interdisciplinary team members to develop individualized care plans based on assessment findings and resident preferences.
    • Monitor and update care plans as needed, incorporating changes in residents' conditions and goals of care.
    • Serve as a resource for staff regarding MDS coding guidelines and documentation requirements.
    • Ensure accurate and timely submission of MDS assessments to regulatory agencies.
    • Participate in quality improvement initiatives to enhance resident care and outcomes.
    • Stay current on industry trends, regulations, and best practices related to MDS and long-term care.
    Qualifications:
    • Bachelor's degree in Nursing or related field preferred.
    • Current licensure as a Registered Nurse (RN) in the state of NY.
    • Minimum of 2 years of experience in long-term care or skilled nursing facility.
    • Certification in MDS completion preferred.
    • Strong knowledge of MDS guidelines, regulations, and reimbursement systems.
    • Excellent communication and interpersonal skills.
    • Ability to work independently and collaboratively within a team environment.
    • Detail-oriented with strong analytical skills.
    • Proficiency in computer skills, including electronic health records (EHR) systems.
