    Your Role as a Home Health Physical Therapist:


    In this rewarding position, you will:

    • Provide home-based physical therapy to patients recovering from illness or injury.
    • Collaborate with a dedicated home health team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to focus on patient care plans and goals.
    • Conduct assessments, document progress, and adjust care plans as necessary.
    • Guide patients through exercises and hands-on therapy to enhance mobility and manage pain.
    • Educate patients and their families about care plans, goals, exercises, and self-care techniques.

    What You Need to Succeed:

    • Graduation from an accredited Physical Therapist Program and an active PT license in Michigan.
    • At least 1 year of clinical experience, preferably in home health within the last 5 years.
    • Current CPR certification.
    • Strong clinical judgment, excellent interpersonal skills, dexterity, and a compassionate nature.
