- Annual salary of $80,000 - $90,000

- Conduct ongoing interdisciplinary patient assessments
- Ensure that our home care clinicians adhere to high standards of safe, effective, and efficient clinical practice through supervision and support in compliance with all relevant laws and regulations
- Supervise professional and paraprofessional team members
- Utilize clinical judgment to manage an assigned patient group effectively and efficiently
- Facilitate patient and family education to support progress toward established goals
- Graduate of an accredited nursing program with an active RN license in Colorado
- At least three (3) years of nursing experience in home healthcare preferred
- Preferably two (2) years of management experience
- CPR Certification (including a demonstration course)
- Familiarity with state and federal home health regulations
- Strong clinical skills, effective communication, and compassion
