- Individualized Case Management Services: Ensure individuals receive ongoing case planning, individualized and personalized goal setting, feedback on progress toward goals, support and follow up for making progress toward individual goals.
- Case Management Coordination: Ensure comprehensive and optimal service delivery for all individuals by serving as the main contact and liaison for the individual and cooperating partners, persons and/or agencies.
- Documentation: Complete and maintain essential documentation about participant needs and the services provided within established timelines and in accordance with applicable procedures, policies and requirements.
- Typical starting pay range: $ $28.00

- Relevant Experience required (1-3 years)
- Associate Degree or Bachelor's degree in a related field preferred
